No Emergency Use

Appointments and Communications

Submitting a form, message, or appointment request does not guarantee an appointment or establish a provider-patient relationship. Vitalief may contact you by phone, email, or other authorized communication methods to respond to your request. SMS messages require the separate consent described in the SMS Program Terms below; submitting contact information alone does not authorize SMS.

SMS Program Terms

You may opt in to SMS through a Vitalief Medical Center Facebook Instant Form by reviewing the SMS disclosure, selecting the separate optional SMS checkbox (unchecked by default), and submitting the form. The Spanish checkbox reads “Acepto recibir estos mensajes SMS.” The English checkbox reads “I agree to receive these SMS messages.” The form may be submitted without selecting SMS consent. Providing a phone number, accepting general terms, sharing contact information, or agreeing to Messenger contact alone does not authorize SMS. Only people who affirmatively select SMS consent authorize texts about their inquiry and appointments through this form. Consent is optional and is not a condition of purchase or care.

Effective September 23, 2026. The Vitalief Medical Center SMS program provides two-way customer support and appointment scheduling assistance through Ava, our virtual assistant, and our team. In addition to Facebook form opt-in, you may initiate a conversation by texting 407-315-1642. Responses relate to your inquiry and may include an appointment confirmation after the appointment has been successfully booked. A request alone is not a confirmed appointment.

By initiating an SMS conversation, you consent to replies about that request and its resulting appointment confirmation. This does not authorize promotional messages, unrelated outreach or recurring reminders. SMS participation is optional and is not a condition of purchasing services or receiving care.

Message frequency varies based on your conversation and scheduling request. Message and data rates may apply according to your mobile carrier plan.

Reply STOP at any time to stop messages from this program. You may receive one final opt-out confirmation; no further program messages will be sent unless you opt in again. Reply HELP for assistance, call 321-800-6123 or email FrontDesk@vitaliefmedicalcenter.com.

Advertising and Online Forms

Vitalief may use online advertising platforms and forms to provide information about our services and respond to inquiries. Information submitted through these tools may be used for internal operations, reporting, and follow-up communication.

Google Ads API Use

Vitalief may use the Google Ads API only for its own internal advertising operations, including reporting, campaign performance analysis, budget monitoring, conversion tracking review, and approved campaign management. Vitalief does not provide Google Ads API access or advertising platform access to third parties through this website.

Accuracy of Information

Limitation of Liability
